About the Role

Sydney Local Health District has a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing care services designed to improve and support the health and wellbeing of individuals and communities.
 
The Out of Hospital Care (OHC) Team Leader manages the operations of the OHC service including management of staff across three sites, achievement of KPIs for annual targets within budget and quality service delivery in compliance with OHC Guidelines and relevant policies. 

This position will be required to work across Sydney Local Health District.

Ideal Candidate

  • Appropriate tertiary qualifications and/or relevant experience in the delivery of aged care, palliative or disability services.
  • High level interpersonal, verbal and written communication skills, including a demonstrated capacity to liaise and negotiate with a wide range of stakeholders.
  • Awareness of relevant government policy, guidelines and initiatives in relation to the Out of Hospital Care client group.
  • Current NSW Drivers Licence (P1 and P2 acceptable).
