Delivering acute care in the patient's own home - Hospital in the Home (HITH) treats acute patients in the home and in the clinic, enabling recovery within their own home and surrounded by family and carers.

Where you'll be working
We are a district wide service and supporting our patients within the Northern Sydney Local Health District. Our team are located primarily at Royal North Shore, Mona Vale and Hornsby. This role will be located at Royal North Shore Hospital campus.
What you'll be doing
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Social Work degree and eligibility for the Australian Association of Social Workers (AASW), including the ability to demonstrate a practical application of the AASW Code of Ethics.
- Demonstrate your understanding of the HITH model of care and what your role as a Social Worker would be.
- Consider the impact and value of visiting acute care patients in their own home and how they interact with their social environment and respond to treatment themselves.
- Explain the key considerations for a social worker who works in a multidisciplinary team delivering acute care - the impact on the person and their family/carers.
