About the role:

The Short Term Residential Accommodation Service (STRAS) unit at Belmont Hospital supports non-acute patients as they transition from hospital to residential aged care. The STRAS team, made up of doctors, nurses, a Diversional Therapist, and placement coordinators, works collaboratively to provide clinical, dementia, and palliative care, while offering emotional support to families during challenging times.

In this role, you will work within a close-knit team, delivering clinical care until a suitable Residential Aged Care Facility (RACF) is found.

Our ideal candidate will be well-supported by the Nurse Unit Manager, Clinical Care Coordinator, and Clinical Nurse Educator. Supernumerary days will be offered, based on your experience and qualifications.



Where you’ll be working:

STRAS is a 20-bed ward dedicated to coordinating admissions into residential aged care following an acute hospital episode. Our goal is to provide temporary care while actively involving the family, patient, and/or carer in finding a suitable facility that meets the patient’s needs. We strive to balance clinical care management with the unique emotional needs of each patient in a supportive environment. Recognizing that the transition from home to aged care can be challenging and unsettling for families, we offer a friendly, professional service to make this process as seamless as possible.

Requirements:

Our successful candidate will possess the following:

  • Enrolled Nurse (Medication Endorsed where applicable) with current Authority to Practice with AHPRA.

 

Additional information:

  • An eligibility list will be created for future permanent full or part time and temporary full or part time vacancies.
  • Interviews are tentatively scheduled for the week commencing Monday 28th October 2024.
  • To be eligible for permanent employment in this position you must be an Australian Citizen, or a permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport.
