Exciting opportunities are available for a qualified and compassionate Social Workers to join the Community Allied Health Team at Central Coast Local Health District (CCLHD). These roles are ideal for a dedicated professional who thrives in a multidisciplinary, community-based environment and is passionate about delivering person-centred care across a wide range of settings.
About the Role
As a Social Worker within Community Allied Health, you will be part of an interdisciplinary team delivering holistic, client-focused care to people in their homes, clinics, aged care facilities, and other community-based settings. You will also support several specialist programs across the district, including the Specialist Palliative Care Service, Chronic Disease Programs, and the Enhanced Community Care programs.
This position plays a vital role in providing psychosocial assessments, counselling, advocacy, and care coordination to patients and their families dealing with the emotional, social, and practical impacts of illness. You will work closely with colleagues and senior leaders to ensure the effective planning and delivery of high-quality social work services.
Key Responsibilities

Provide comprehensive psychosocial assessments and therapeutic interventions for patients and carers.

Deliver case planning, referral, and advocacy services to connect clients with appropriate community supports.

Work collaboratively with interdisciplinary teams to support integrated care delivery.

Participate in service development, quality improvement, and service evaluation activities.

Supervise and mentor social work students and contribute to staff education initiatives.

Provide social work services across various settings, including home visits, residential aged care facilities, and clinics.

About You
- Qualification of a Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers (AASW) (overseas qualifications must supply the AASW Overseas Assessment number) with demonstrated understanding of the current AASW Practice Standards as they relate to the position and a commitment to maintaining a professional skill and knowledge base.
- You have a unrestricted Drivers Licence.
- You have a well-developed understanding of the psycho-social impact of acute and chronic health conditions and in the range of related social work interventions required in community social work clinical practice.
