What you'll be doing

The Sustaining NSW Families (SNF) Clinical Nurse Consultant (CNC) is responsible, together with the SNF Clinical Coordinator, for the clinical leadership of the SNF nurse-led home-visiting program offered, across Northern Sydney Local Health District (NSLHD), to vulnerable families from pregnancy until their child is two years old. (The SNF program is a nurse-led multidisciplinary program for families experiencing low to moderate psychosocial stressors).
The CNC is a Child and Family Health Nurse (CFHN) with advanced practice skills, whose clinical role includes supporting the CYF SNF Clinical Coordinator with new family referrals to the SNF Service, this includes intake, undertaking initial client (family) engagement and clinical assessments, and providing CFHN clinical consultation for team members, leading case reviews and clinical practice improvement projects.
The NSLHD Brighter Beginnings SNF Program operates as part of the broader NSLHD CFHN service supporting parents to optimise wellbeing and their capacity to care safely for their children and to optimise their children’s health, well-being and development, in the first 2000 days of life from conception, to give children the best possible start to life before they start school. The SNF service offers eligible families sustained nurse home-visiting with structured interventions mapped to the child’s age and stage of development. The SNF Program aims to support healthy parent-child relationship development and new parents' capacity to optimise their infant's, physical, emotional, cognitive, communication, and social development. The CNC will support the SNF team to provide a high standard of safe, evidence-based, effective, and person-centred care for infants and their caregivers. The CNC position will use their advanced knowledge and skills to build capacity for all SNF CFHN to ensure consistent standards of care delivery across service locations to maintain the fidelity of the SNF model of care. The position supports the standard of client-care in their team and will ensure that healthcare is safe, integrated, compassionate, person-centred, trauma-informed, and provided in partnership with families and carers. The CNC will be available to support staff with complex clinical care situations, including child wellbeing concerns and case reviews as well as clinical supervision.

 Candidates will need to meet the following criteria:

  1. Registered Nurse with Australian Health Practitioner Regulation Agency (APHRA) with at least 5 years post registration experience; post graduate experience with 3 years full time equivalent in clinical practice in Child & Family Health Nursing and post-graduate qualification in Child & Family Health Nursing (CFHN) or evidence of working towards Masters level qualification.
  2. Demonstrated clinical expertise at an advanced practice level and proven ability to provide clinical leadership in the provision and development of contemporary practice by utilisation of evidenced based research with a demonstrated understanding of current NSW Ministry of Health Policies and National and State Standards related to Child and family Health Nursing including NSW Health Supporting Families Early policy, Sustaining NSW Families and First 2000 Days Strategic Framework.
  3. Demonstrated ability to develop, lead, implement and evaluate evidence-based practice initiatives; education and quality improvement programs; and practice development programs in line with appropriate professional frameworks.
  4. Demonstrated effective communication including verbal, written and interpersonal skills and advanced information technology skills to provide complex consultancy services and influence innovative nursing practice.
  5. Current  NSW Drivers Licence and willingness to drive/work across the LHD.
