Sydney Local Health District (SLHD) is seeking a dynamic Registered Nurse to join our Access Care Team at Concord Repatriation General Hospital!
About the Role
Concord has built on its strengths of providing quality health services to returned servicemen and women and local communities, as well as undertaking world class clinical education and research.
The Registered Nurse is responsible for delivering person-centred, evidence-informed care that supports positive health outcomes. Working in collaboration with individuals and the multidisciplinary healthcare team, the nurse will assess, plan, implement, and evaluate nursing care.
We are currently seeking experienced Registered Nurses to join the Access Care Team (ACT), supporting the delivery of intake services for Aged Health, Chronic Care, and Rehabilitation programs. ACT also provides telephone triage and telehealth support to residential aged care facilities, assisting in the care and management of their residents.
To be considered for this role you must have…
- Current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA).
- The ability to participate in a seven day rotating roster
- A high level proficiency in the use of computer software applications is essential for this position
- Evidence of participation in own professional development and commitment to maintaining competence and enhancement of skills, and willingness to apply and share professional expertise with colleagues.
