Where you'll be working
Population and Community Health (PaCH) is part of South Eastern Sydney Local Health District.
PaCH provides community health services that give care to individuals, families and carers, as well as population health services that focus on the whole community or key groups within the community.
PaCH's services are provided at the District's hospitals, community-based facilities including child and family health centres, community health centres and specialist clinics. We work with other service providers across South Eastern Sydney Local Health District, including hospitals and mental health services.
The Clinical Assessor undertakes a complex aged care assessment using the Integrated Assessment Tool (IAT) addressing the medical, physical, cognitive, psychosocial, cultural, environmental and social aspects of older people referred for assessment via the My Aged Care Contact Centre. The Clinical Assessor formulates a support plan that best meets the needs of the older person and their carer within the resources available.
The Clinical Assessor works within the guidelines set by the Single Aged Care System Assessment Services Contract Statement of requirements, the My Aged Care Assessment Manual 2024 and the Aged Care Act 1997.
The Clinical Assessor also conducts Residential Aged Care (RAC) Funding Assessments using AN-ACC (Australian National Aged Care Classification) Assessment Tool in My Aged care System (applies to Physiotherapists, Occupational Therapists and Registered Nurses with a minimum of five years qualified only).

Selection Criteria
- Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award or Public Health System Nurses and Midwives (State) Award and current registration with a relevant Registration Board (AHPRA), or membership or eligible for membership of relevant recognised profession body and demonstrated commitment to ongoing professional learning.
- Demonstrated core competency skills in comprehensive assessment of frail older people as community clients or inpatients.
- Demonstrated high level written, verbal and interpersonal skills, to both engage patients/clients and work effectively with the multi-disciplinary team.
- Demonstrated ability to proactively engage with patients/clients to enhance service delivery and to guide others in providing service excellence.
- Demonstrated ability to work as part of a multi-disciplinary team and apply professional and ethical boundaries when dealing with complex situations.
- Proficient skills in professional and clinical digital technologies i.e. eMR, CHOC, My Aged Care, Microsoft Office applications.
- Detailed knowledge and understanding of risk management, safety and quality principles and the role of the Clinical Assessor in applying these principles.
- Current unrestricted NSW drivers licence and willingness to travel in accordance with the demands of the position.
