About the role:

This role will be based in Tamworth, but will also serve the New England North West. Your core focus will be care coordination and service navigation - helping individuals with chronic and complex health needs access the right care at the right time. You will communicate predominantly via phone, having a direct impact on reducing hospital admissions and improving wellness at home. Ongoing training, development, and peer support are available within the broader Social Work network.

Working alongside a team of experienced nurses, you will:
 

  • Support hospital avoidance strategies, reducing unnecessary Emergency Department (ED) presentations.
  • Collaborate with ED clinicians and paramedics to identify patients who frequently present and address service gaps contributing to their presentation.
  • Conduct short-term case management for clients with chronic and complex needs.
  • Liaise with a wide network of services including NDIS, public and private providers, allied health, and community supports.
  • Refer clients to appropriate services and follow up to ensure continuity of care.

 
About you:

We're looking for someone who is:
 

  • Experienced in health or hospital settings, ideally with knowledge of chronic and complex care management.
  • Skilled in remote and phone-based service delivery.
  • Confident working across services and systems to identify needs and coordinate care.

 

Requirements:

  • Current NSW Drivers Licence and willingness to travel
  • Must hold a minimum bachelor degree in social work which provides eligibility for membership of the Australian Association of Social Workers


Additional Information:

  • To be eligible for permanent employment in this position you must be an Australian Citizen, or a permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport. Employment of a temporary visa holder may only occur if no suitable local candidate is identified for the position; in this instance, you may only be offered employment in line with the conditions and expiry date of your visa.
