About Your Role
The Rural Generalist Nurse Practitioner will be centralised to a cluster within the Local Health District to provide outreach support i.e. Integrated care with Primary Health Network, Emergency, Acute Care, Ageing and Palliative Care, Chronic and Complex Care, Mental Health Care, Women’s Health and Child and Family Care. These roles will function both within and across the sites and services within these clusters.
Rural generalist NPs need to be able to manage a broad range of acute and chronic presentations across the lifespan to address patient and community needs. NP will receive and make referrals to facilitate integrated care across disciplines or teams.
The NP model will focus on the delivery of accessible, person-centred care in rural settings particularly where there is limited medical cover or those reliant on Virtual Care. The NP will adhere to existing governance and policy requirements for NPs in NSW.
The NP will create culturally safe partnerships to address priority populations in their clusters including the following communities: Aboriginal and Torres Strait Islander people, those from refugee and asylum-seeking backgrounds, people living with a disability, people experiencing homelessness or on low incomes, older people and culturally and linguistically diverse groups.
The NP model can enhance care delivery through:
- Assessment and treatment
- Telemonitoring of signs and symptoms
- Telehealth services
- Providing outreach to spoke services and clinics
- Symptom and medication management
- Discharge follow up and education
- Health promotion and prevention
- Support for patient/clinician enquiries aimed at reducing re-presentations and increasing patient satisfaction.
The Nurse Practitioner will provide high quality, evidence based care at an advanced and extended nursing level to achieve quality care outcomes for patients and clients.
