An exciting opportunity exists for an experienced Renal Transplant Clinic Pharmacist to establish a new role with a dynamic multi-disciplinary team at Royal North Shore Hospital.

What you'll be doing
The Renal Transplant Clinic Pharmacist is a 1 day/week, 12-month (temporary) position.
The Renal Transplant Clinic Pharmacist will deliver pharmaceutical care to patients in the Acute Unstable Transplant Clinic as well as renal transplant inpatients at Royal North Shore Hospital. They will:
- Provide specialised clinical review, patient counselling and tailored medication management for renal transplant patients.
- Provide collaborative care as part of the multidisciplinary transplant team to ensure holistic management of transplant patients.
- Develop and implement evidence-based medication protocols to enhance service delivery and patient outcomes.
- Establish and embed a high-quality clinical pharmacy service model including data metrics to demonstrate cost effectiveness, efficiencies and role sustainability.
- Contribute to other quality improvement and research activities, in collaboration with RNSH Pharmacy Department and Department of Renal Medicine.
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Current registration as a pharmacist with the Pharmacy Board of Australia (or eligible for immediate registration) with no conditions or restrictions on practice and evidence of extensive and relevant hospital experience.
- Demonstrated ability to initiate, lead, complete and share quality improvement activities or service evaluation process. Experience in clinical teaching and supervision of students, interns, Pharmacist, and/or Pharmacy Technicians/Assistants.
- Demonstrated proficiency in ability to work autonomously with limited supervision. Have the ability to prioritise work, manage a demanding workload, apply high level reasoning skills and have independent professional judgement.
- Extensive experience in the provision of advanced clinical pharmacy services including demonstrated advanced level knowledge and clinical practice in renal transplant. Experience delivering clinical pharmacy services to outpatient clinics is preferable.
