  1. Relevant qualifications as a Registered Nurse, Occupational Therapist or Psychologist currently registered with the Australian Health Practitioner Regulation Agency (AHPRA), or qualified Social Worker eligible for registration with the Australian Association of Social Workers (AASW), or CSU Graduates: Bachelor of Health Science (Mental Health).
  2. Please state your current Australian residency status. If on a temporary visa, please let us know its conditions and expiry. 
  3. Please provide a detailed description of your interest and motivation in applying for this role, highlighting your relevant skills & experience in providing assessment and treatment for consumers of the mental health service.
  4. Using an example, describe a time where you have had to work as part of a team to solve a problem. Outline the communication skills used and how you ensured the problem would not occur again.
  5. Must possess a valid Australian Drivers Licence (excluding learner driver's licences). If not already in possession, a NSW Drivers Licence must be obtained within 3 months of appointment.
