Where you'll be working:

Join us at Hunter New England Health’s Cardiac and Pulmonary Rehabilitation Service – delivering best-practice care to patients with acute and chronic cardiac and respiratory conditions. You'll be based in a supportive, multidisciplinary team working across a variety of community and outpatient settings, with access to senior clinical guidance and flexible work models.


About the role:

This dynamic role offers the opportunity to provide safe, person-centred physiotherapy treatment to clients with cardiac and pulmonary disease. You'll conduct comprehensive assessments, develop individualised exercise prescriptions, and deliver both group-based and 1:1 rehabilitation programs, including home visits and outpatient consults.

You’ll also:

  • Co-facilitate multidisciplinary rehabilitation programs alongside a Registered Nurse

  • Educate, coach, and empower clients to self-manage their condition

  • Use evidence-based approaches to improve outcomes and reduce hospital admissions

  • Work across a blend of clinic-based, home-based, and virtual care models

  • Deliver care aligned with HNE Health’s CORE Values and Excellence Framework


About you:

We’re looking for a motivated, compassionate physiotherapist who is:

  • Passionate about improving the lives of clients living with chronic disease

  • Skilled in tailoring exercise and education programs to a wide variety of needs

  • Empathetic, approachable and confident in building rapport

  • A strong communicator who thrives both independently and within a team

  • Committed to continuous improvement and professional development

  • Experienced in delivering safe, evidence-based interventions


Requirements:

  • Tertiary qualification in Physiotherapy

  • Current registration with AHPRA

  • Current NSW Driver’s Licence (and ability to travel for home visits/outreach)

  • Experience in cardiac or pulmonary rehabilitation (desirable, but not essential)
