What You'll be Doing
The Nurse Manager Patient Flow is responsible for ensuring seamless patient flow management within the facility through the coordination and facilitation of patient flow strategies to improve the delivery and timeliness of access to care. This role will ensure optimal patient outcomes by promoting best practice through facility capacity / demand management processes.

This position reports directly to the Demand Manager, works in collaboration with the demand Management Unit and both internal and external stakeholders to maintain access and flow across the facility. Responsible for coordinating and facilitating patient flow strategies to improve the delivery and timeliness of access to care

The role will provide guidance and support to ensure that there is a focus on service development and appropriate oversight of practices, trends and indicators. Ongoing assessment and improvement in operational processes and clinical outcomes is an inherent responsibility of this position. The Patient Flow Manager is to foster and maintain close professional relationships with relevant executive teams, clinical managers, peers and district managers to achieve organisational goals. This position will support the SWSLHD Strategic Plan as well as the facility/service operational and/or clinical service plan and any relevant service operational or strategic plans.

  1. Current Authority to Practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA) and Relevant tertiary qualifications or equivalent work experience or a combination of study and experience deemed appropriate by SWSLHD.
  2. Work experience with a sound understanding of hospital, health service administration and demand management practices.
  3. Demonstrated senior nursing experience with leading staff and ability in delivering initiatives in a relevant setting.
  4. Demonstrated ability to provide leadership as a resource person and role model through good communication and the development of professional relationships.
  5. Demonstrated ability to utilise a broad range of communication, delegation and negotiation skills in a variety of settings, ensuring that you are able to effectively collaborate with staff and other health professionals for the best outcomes of the service.
  6. Analytical skills and an aptitude to utilize relevant information technology platforms including the PFP, FirstNet and Microsoft Office Suite of programs.
  7. Demonstrated expertise in organizational performance measures and their relationship with strategic decision making and driving change.
  8. Extensive experience with leading and embedding projects, strategies and or change initiatives utilizing evidenced based practice and a change methodology.
  9. Demonstrated knowledge of the Whole of Health Improvement program and ability to coach and support change management within the facility.
