The MLHD Out of Hospital Care (OHC) Coordinator will coordinate all aspects of the OHC program. The OHC program includes ComPacks, Safe and Supported at Home (SASH) and End of Life (EOL) packages. 

The OHC Coordinator will ensure that all operational aspects of the OHC program within the LHD and facilities are managed effectively including identification of patients; prioritisation of referrals; monitoring of availability; assessment of eligibility and monitoring all daily operational aspects of the program.
