An exciting opportunity to join a new program providing face to face specialist Palliative Care Nurse Practitioner support 4 days per week (Friday to Monday) for patients, carers and families where timely intervention is required to support patients with distressing symptoms and rapid deterioration.

Where you'll be working


Care will be provided to patients in the community residing in the Northern Sydney Loal Health District (NSLHD) catchment area. The clinicians will be based at Macquarie Hospital North Ryde.

What you'll be doing

The Palliative Care Nurse Practitioner (NP) is embedded withi1n the Northern Sydney Home Nursing Service (NSHNS) providing face to face specialist nursing support 4 days per week (Friday to Monday)  for patients, carers and families where timely intervention is required to support patients with distressing symptoms and rapid deterioration. The NP undertakes expert rapid assessment , intervention and/or medication titration to avoid unnecessary  acute hospitalisation, assisting people to stay at home or their preferred place of care, reducing stress on families and carers at end of life. The role allows a rapid response to urgent requests and initiation of end-of-life plans of care.

The NP position enhances the current after-hours telephonic support provided by the Hammond Care Community Palliative Care Service and compliments the anticipatory care model already provided by the community nursing service, Northern Sydney Home Nursing Service. The focus of the position provides first line specialist virtual and face to face palliative care assessment, advice and care planning across weekends and public holidays to people in the community. The NP will have a strong interface with the NSLHD Virtual Care Service Palliative Care Clinical Nurse Consultant Navigation service to respond to NSW Ambulance calls for support and the ability to access to Support at Home End of Life Care (EOL) Packages.

The role works in collaboration with the on-call palliative care specialist medical staff, escalating concerns and seeking advice where complex management issues are outside the scope of the Nurse Practitioner practice and expertise.

Candidates will need to meet the following criteria
:

  1. A Registered Nurse appointed as such to a position approved by the Secretary NSW Health and who is endorsed by the Board, to practise as a nurse practitioner.
  2. Holds a current NSW driver’s licence, enabling flexibility and mobility for community-based care.
  3. Proven clinical expertise and competency in the management of patients requiring Supportive and Palliative care, with a track record of delivering high-quality, person-centred care.
  4. Demonstrated ability to work autonomously whilst effectively contributing to community nursing services and engaging in multidisciplinary collaboration to achieve optimal patient outcomes.
