We are seeking an experienced Nurse Practitioner to join our Aged Care Outreach Service team. As the successful candidate, you will play a key role in providing timely, high-quality care to residents in Residential Aged Care Facilities (RACFs).

 

The role will focus on offering acute care treatments at the residents’ homes, enabling them to remain in their familiar surroundings while receiving the best possible care. Your responsibilities will include assisting in assessing, diagnosing, and managing acute deteriorations in RACF residents, reducing emergency department presentations, and promoting hospital avoidance strategies to minimize complications with other members of the multidisciplinary team.

 

In addition, you will actively contribute to upskilling RACF nursing staff through education on common aged care conditions. You will collaborate closely with GPs and other healthcare professionals to ensure continuity of care and facilitate direct hospital admissions when required, bypassing the emergency department. This position will be primarily in the Wollongong office however also may be required to work in the Shoalhaven.

Wollongong Hospital, located on Dharawal Country, is the Illawarra Shoalhaven’s major referral and teaching hospital and has a bed-base count of more than 500.
