ABOUT THE ROLE
The Manager Complex Care and Frequent User will manage the development, implementation, monitoring, support and evaluation of clinical programs that improve the level and quality of complex care service provision, including patients who meet requirements for the Frequent User Management Program, Complex Care and / or Authorised Care Plan Program at NSW Ambulance. The role will be required to lead a multidisciplinary team and work collaboratively with a range of internal and external stakeholders to develop and deliver innovative projects and models of care

ABOUT YOU
Our ideal candidate is a demonstrated people leader with a proven track record in building and maintaining strong, cohesive teams and in managing, leading and supporting small teams through a period of major change. The candidate will need to be experienced working within multidisciplinary teams (both internally and externally) to identify, develop, implement, monitor and evaluate projects that improve complex care service provision, ensuring high quality outcomes and deliverables within agreed timeframes
The successful candidate will have demonstrated experience in clinical practice that reflect patients' bio-psycho-social needs and experience working with psycho-socially vulnerable individuals who experience challenges engaging with health care providers.
