- Applicants must be of Aboriginal descent through parentage, identification as being Aboriginal and accepted in the community as such. Aboriginality is a genuine occupational qualification and is authorised under Section 14(d) of the Anti-discrimination Act 1997;
- Must possess a Certificate IV in Aboriginal Primary Health Care (Practice) and a current authority to practice as an Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA) and current NSW Driver’s Licence and willingness to drive/travel state-wide as required;
- Demonstrated understanding of issues affecting Aboriginal people in relation to grief and loss and Aboriginal social and emotional wellbeing and understanding of Cancer Care, Advance Care Planning and Palliative Care Aboriginal services in the community;
- Demonstrated experience in coordinating/case managing the care of Aboriginal patients with Cancer or Palliative Care and/or End of Life Care and capacity to lead the development of culturally safe environments for Aboriginal patients and their families within a multidisciplinary team;
- Effective interpersonal skills, problem solving skills and personal resilience
- Demonstrated ability to liaise, proactively engage and work collaboratively with vulnerable communities and relevant agencies in ensuring culturally responsive services;
- Demonstrated proficiency in using computer systems in a Windows environment, including MS Office applications and common database/statistics software;
- Demonstrated understanding of the NSW Health Integrated Care Strategy and a commitment to the values of the Justice Health and Forensic Mental Health Network.
