Integrated Care Department at SCHN is expanding. The team is fostering integration between individuals, processes and systems to enhance human experience and achieve better outcomes for children, young people and families.
What you'll be doing
Integrated Care team is fostering systems integration within SCHN and at the interface with social care. The team is enhancing integration between people, processes, models of care and enablers. The team is fostering integration across systems to enhance experience and achieve better outcomes for children, young people and families, enhance experience for staff.
The position will provide advanced clinical experience, project management and change management expertise to achieve integration of the clinical teams around the individual patient and family level; and at the broader service, organisational and system levels to improve experience and efficiencies.
- Relevant tertiary qualifications in health or clinical work experience, or a combination of study and clinical work experience.
- Demonstrated experience as a clinician in project management and change management in a health setting, and/or clinical redesign experience, with achievement of planned outcomes.
- Demonstrated problem solving skills with capacity to successfully manage diverse, complex, and competing priorities and meet deadlines.
- Demonstrated skills in consulting with a wide range of clinicians and supporting clinicians with quality improvement.
- High level interpersonal communication, verbal and written skills and demonstrated ability to communicate, consult and negotiate effectively with internal and external stakeholders.
- Demonstrated computer literacy to support project management requirements.
- High level analytical skills including ability to collect, analyse, interpret, and report on complex data and information from numerous sources.
- Current unencumbered Drivers Licence for use within NSW.
