The Role
We’re currently looking for a Clinical Nurse Specialist to coordinate Hepatitis linkage to care to the General Practitioners project in the South Eastern Sydney and Sydney Local Health District catchment areas.
The position will manage all aspects of the project including identification of patients released in the catchment area who require on-going care for Hepatitis and Advanced Liver Disease, telehealth appointments prior to release, referral to General Practitioners and data collection. The position will coordinate patient assessments and facilitate communication between the patient and the General Practitioners and Hepatitis NSW peers as required.
The position will monitor and manage data integrity and collection against outcome measures for reporting to the Central Eastern Sydney Primary Health network on a quarterly basis.
