What you'll be doing- Provide care coordination, case management and navigation for people with a disability who are in hospital to ensure they have appropriate supports for discharge which would ensure a successful discharge from hospital.
- The Disability Navigator is the Specialist Intellectual Disability Health Team (SIDHT) Outreach Clinician for the FWLHD and is expected to perform the same duties expected of a SIDHT Clinician.
- Support NDIS eligible people who are in hospital, at risk of hospitalisation or where the support will expedite access to NDIS services to relieve pressure on FWLHD services.

How to Apply
To be considered for this position, please ensure you address the selection criteria in the application as thoroughly as possible
- Tertiary qualification and clinical experience as an Allied Health Clinician preferably in Social Work or a relevant Allied Health profession, with eligibility for registration with the appropriate professional body.
- Demonstrated advanced clinical reasoning, experience and practice skills as relevant to assessment and treatment of people with disability.
- Demonstrated experience working in the disability sector, with a strong understanding of navigating the NDIS and alternative disability related supports.
- Demonstrated ability to coordinate and communicate complex care plans within a health care context, as part of a multi-disciplinary team.
- Demonstrated experience of effective negotiation, advocacy, collaboration and communication skills with a wide range of stakeholders.
- Demonstrated ability to prioritise caseload, manage and respond to stressful situations and the ability to work autonomously.
- Ability to complete reports utilising language relevant and appropriate for the Government agency to give people the best opportunity to obtain supports.
- Current unrestricted NSW drivers’ licence. The ability, and willingness to travel across the Far West LHD, frequently staying overnight, sometimes several nights
