$36.39 - $53.17
per hour (dependent on qualifications) + superannuation + salary packaging

About the role:

A typical day or week in this role will see you working within our Diabetes Alliance Program Plus (DAP+), where you will:

  • Provide integrated diabetes specialist care to patients diagnosed with diabetes (type I, type II and gestational) within the general practice setting, including face-to-face and virtual case conferencing modalities.  
  • Provide diabetes education to primary care colleagues and patients as appropriate, and collaborate to provide patients with the best possible care. 
  • Work with Aboriginal Medical Services (AMS) and displaying a commitment to working alongside Aboriginal and Torres Strait Islander patients, families and employees to enhance health outcomes for Aboriginal and Torres Strait Islander clients located within Hunter New England communities.
  • Travel to our face-to-face case conferencing clinics within general practices located across and within the Hunter New England regions.
  • Work within a multidisciplinary team, including Managers, Diabetes Specialists, Diabetes Educators, Aboriginal Health Workers and Administration Officers. 


You will be supported within the DAP+ clinical team and report directly to the program Implementation Manager.  Ongoing learning and training opportunities are provided, including learning modules offered within the HNE My Health Learning platform and HETI.

Where you’ll be working:

DAP+ is a dedicated initiative aimed at enhancing health outcomes and patient care experiences for individuals with diabetes, all within the convenience of their general practice setting.  DAP+ provides a variety of flexible and innovative services, including in-person and virtual case conferencing, diabetes and endocrine advice line, education events and conferences, and mobile Medibus. 

This role is based at Wisteria House on the James Fletcher Campus in Newcastle.

About You:

We are looking for someone with a current Credentialed Diabetes Educator qualification, or willingness to work towards. You should have exceptional interpersonal, communication, and verbal skills, as well as sound knowledge and experience within both the primary care and/or hospital sectors.

This role will require you to be able to adapt to any environment and work both collaboratively with others, and independently when necessary.  

Requirements:

  • Must hold a qualification in one of the following: Nursing, Nutrition and Dietetics, or Physiotherapy which provides eligibility or membership of the relevant professional association general registration through the Australian Health Practitioner Regulation Agency.
  • Eligibility to drive in NSW and the ability and willingness to travel for work purposes which may involve driving long distances


Additional Information:
 

  • To be eligible for employment in this position you must be an Australian Citizen, or permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport, or hold an appropriate visa that allows you to live and work in Australia. Employment of a temporary visa holder will only be offered in line with the conditions and expiry date of your visa. 
  • An eligibility list will be created for future temporary part time vacancies.
