Clinical Nurse Specialist - Wellbeing

About the role:

  • The Wellbeing Nurse CNS2 is responsible for facilitating identification of health, wellbeing and social needs and the coordination of appropriate early intervention, assessments, and referral to services for school-aged children and young people attending three (3) Muswellbrook schools – two primary and one High School.
  • The Wellbeing Nurse CNS2 is a Registered Nurse who works in the schools and applies their specialist clinical nursing knowledge, experience and skill in providing complex health care coordination for school aged children, young people and their families in these schools.
  • The Wellbeing Nurse is supported by the Nurse Unit Manager at Hunter Valley Community Health, as well as the other WHIN coordinators within the District.

Where you'll be working:

  • You will be working within Hunter Valley Community Health team, based at Muswellbrook Community Health Centre.
  • Primarily, you will be working at each of the nominated schools on a day-to-day basis.
  • The HVCH Nursing team has a variety of other disciplines - Generalist Nursing, Child and Family Health, Palliative Care, Women's Health, Dementia Advisory Service, Families NSW Perinatal Service.

About you:

  • The Wellbeing Nurse CNS2 role requires leadership, specialist skills, capacity for a significant level of autonomy and independent work in an interagency context.
  • Coordination of access to services across multiple agencies, professions and groups will be part of the role.

Requirements:

  • Current Registered Nurse with AHPRA, with a minimum of 3 years post registration clinical experience working with children and young people, a post registration qualification related to the specialty area of child and family health, or paediatrics, or adolescent / youth health nursing practice or other related area.
