About Us

The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

An exciting Permanent Full time opportunity is now available with the Primary Care and Community Health Unit. We are seeking a Registered Nurse who applies a high level of clinical nursing knowledge, experience and skills in providing complex nursing care with minimum direct supervision

The role of the Clinical Nurse Specialist is responsible for clinical intervention and co-ordination of a client’s care as part of the Community Supportive Care team. within the Enhanced Community Care (ECC) Model of Care.  The primary purpose of this role within the interdisciplinary Community Supportive Care team is to work with other community health teams, general practitioners, hospital teams and specialist palliative care teams to co-ordinate and deliver best supportive care to clients with late stage degenerative and chronic conditions and disability. The goal of the Community Supportive Care team is to promote and maintain a client’s function and independence at home, and to provide support for their family and carers.

What you will bring to the role

  1. Current registration to practice as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Authority (AHPRA).
  2. Post-registration qualifications (Grad Cert minimum) in Palliative Care and/or relevant specialty with at least 3 years full time equivalent experience working in supportive and palliative care nursing, or such other qualifications or experience deemed appropriate by the organisation.
  3. Commitment to champion, educate and lead an interdisciplinary team to improve clients’ outcomes as part of a team.
  4. Demonstrated problem solving and decision-making abilities according to best practice.
  5. Demonstrated ability to communicate effectively with clients, carers and a diverse range of health care providers.
  6. Demonstrated commitment to clinical practice development.
  7. Ability to work independently with minimal supervision maintaining involvement within the wider interdisciplinary team.
  8. Current Driver's Licence and willingness to use for work purpose travel. This may include driving long distances across the LHD.
