The Role

The Primary Care Clinical Nurse Specialist 2 will deliver care to adult patients who suffer from acute or chronic illnesses in the custodial setting. This position is required to interface effectively with staff, managers and other service providers within Justice Health and Forensic Mental Health Network (Justice Heath NSW), Corrective Service New South Wales (CSNSW) custodial and program staff and external service providers. The position holder will work within Justice Health NSW and NSW Health policies, procedures, and guidelines as well as CSNSW rules and regulations as set out in the relevant legislation.

 
The position provides support to Justice Health NSW clinicians to assist with the continuity of care, case management, transfer of care into the community, post release follow up as required for patients and responsibility and accountability for planning, development, implementation and evaluation of primary care services, including the co-ordination and monitoring of service provision.
