About you
Must hold an eligible membership of the Australian Association of Social Workers; or qualifications recognised for registration with the Occupational Therapy Board of Australia; or qualifications recognised for registration with the Physiotherapy Board of Australia; or qualifications eligible for registration with the Psychology Board of Australia; or qualifications eligible for registration with the Nursing and Midwifery Board of Australia.
Can you demonstrate an ability to conduct comprehensive clinical aged care assessments and problem solve complex cases with a focus on achieving positive client and carer/family outcomes while ensuring a person centred approach within a multidisciplinary team environment?
Have you worked with aged clients for a minimum of 12 months on a ward, in an aged care residential setting or in nursing or allied health services / clinics?
What you'll be doing
To provide safe, compassionate and high quality person centred comprehensive aged care clinical assessments to clients.
Ensuring eligibility for aged care services in collaboration with clients, their families/carers and a multidisciplinary team to achieve identified goals and outcomes in line with MNCLHD Aged Care Assessment Service guidelines (ACAS).
