Chronic and Complex Care Coordinator
- Current registration as a Registered Nurse with the Australian Nurse Health Practitioner Regulation Authority (AHPRA) with relevant post-registration qualifications and at least 3 years' experience working in the clinical area of their specified post-graduate qualification or such other qualifications or relevant equivalent experience.
- Experience in practice that promotes self-management support, behaviour change and psychosocial support and a strong understanding of the health and psychosocial issues for people living with chronic conditions and their carers.
- Demonstrated ability to communicate effectively with patients, carers, and a diverse range of health care providers including those in government, non-government, and community organisations.
- Substantial and current experience in Complex Care Coordination, using advanced clinical skills in patient assessment and care planning.
