Central Coast Local Health District (CCLHD) are seeking a Nurse Manager Grade 3. Are you a dynamic leader ready to join our multidisciplinary team as an After Hours Patient Flow Manager? If you are passionate about making a difference this is your opportunity to showcase your skills.

About You 

  • You have current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse (Division 1) and current NSW Driver’s Licence.
  • You possess demonstrated extensive nursing and management clinical experience in the acute setting with the ability to work autonomously and manage conflicting priorities to meet deadlines in a high volume / pressure environment while demonstrating a focus on service excellence through the ability to critically analyse and act on issues quickly, decisively and effectively.
  • You have demonstrated understanding of the complexity in managing patient flow and the systems and processes related to patient flow and patient transport; and a knowledge of the clinical services available in CCLHD as well as high-level communication and negotiation skills with Senior Managers and clinical staff. Excellent written/oral communication and interpersonal skills and experience in the preparation of complex correspondence and reports.

Where you'll be working

The After-hours Patient Flow Manager (AHPFM) is responsible for coordinating and managing the movement of patients in and out of facilities within the CCLHD and other LHD’s. 

The AHPFM is responsible for monitoring Ambulance Transfer of Care (ToC) as the CCLHD delegate and act as the Executive Level contact for ToC communication escalation between NSW Ambulance and the Ministry of Health (MoH). 

The AHPFM will work in collaboration with site AHNM, Patient Flow units and Patient Transport Services (PTS) to co-ordinate patient flow ensuring timely access to patient care and build constructive relationships within the organisation.
