Case Manager, Transitional Aged Care Service - Tweed Community and Allied Health

1. Occupational Therapist / Physiotherapist / Social Worker / Dietitian / Speech Pathologist – relevant tertiary qualifications and registration with Australian Health Practitioner Regulation Agency (AHPRA) or eligible for membership with AASW / DAA / SPA as appropriate; or Registered Nurse (Division 1) with the Nursing and Midwifery Board of Australia with a postgraduate qualification in Aged Care; and at least 3 years experience
2. Demonstrated high levels of experience providing comprehensive needs assessment, care and service planning and evaluation for patients with complex needs in partnership with clients, their carers and service providers.
3. Evidence of clinical leadership skills and the ability to work collaboratively within a multidisciplinary framework to plan, deliver and coordinate patient care.
4. Demonstrated ability to apply advanced clinical expertise and independently provide case management that supports good patient care outcomes, within the aged care and health sectors.
5. Demonstrated high level knowledge and understanding of person-centred care, quality improvement and evidence based practice.
6. Demonstrated commitment to excellence in practice relevant to qualifications, professional development and the provision of education and supervision to others.
7. Demonstrated effective communication through the use of information technology, written, verbal and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others.
8. Valid unrestricted drivers licence for use in NSW/Australia and willingness to travel in the course of employment.
