About the role


The Wellbeing Nurse Coordinator works within the identified schools to support children and families through coordination of health care.

 

The Wellbeing Nurse Coordinator works as the link between the school and the health care system by navigating the journey for identified children.  This role focuses on coordination of health care to support children who have been identified at school as needing support or specialist’s appointments.  

This role will be supporting selected schools within these communities.

  • Service Delivery Directorate
     

As the Wellbeing Nurse Coordinator you will be working closely with the schools learning and support and leaderships teams to support healthcare services for children and young people. 

You will also be working part of the wider wellbeing team within the LHD. 

The role works as part of the Service Delivery Directive.
