Selection Criteria

  1. Current Registration with the Australian Health Practitioners Regulation Agency (AHPRA) (Nursing, Occupational Therapist, Physiotherapist) or eligibility for membership with the peak professional association (Social Workers, Dietitian, Speech Pathologist).
  2. For Nurses, 5 years post registration experience plus 2 years working with frail older consumers and those with chronic illness. For Allied Heath clinicians, demonstrated extensive specialist knowledge of frail older consumers and those with chronic illness.
  3. Demonstrated knowledge and understanding in applying research and evidence-based practice to the workplace with the ability to think critically, problem solve and link outcomes to the principles of continuous quality improvement in the clinical speciality field.
  4. Demonstrated high level of interpersonal and communication skills, including written, verbal and computer skills and an ability to work collaboratively in a multidisciplinary team.
  5. Proven clinical leadership capabilities with the ability to provide consultancy advice, education and support to individuals, their families, allied health and nursing staff, and other health professionals.
  6. Demonstrated ability to lead family/ case conferences, discharge planning and coordinate transfer of care for complex presentations across the continuum of care. This includes a demonstrated knowledge of the eligibility criteria and referral process for post discharge options of care.
  7. Current unrestricted drivers' licence and the ability to maintain. Ability and willingness to drive/travel as the role requires. This may involve driving long distances.
