Virtually Enhanced Community Care - Transition of Care Coordinator - Site Neg within SNSWLHD
Selection Criteria
- Current Registration with the Australian Health Practitioners Regulation Agency (AHPRA) (Nursing, Occupational Therapist, Physiotherapist) or eligibility for membership with the peak professional association (Social Workers, Dietitian, Speech Pathologist).
- For Nurses, 5 years post registration experience plus 2 years working with frail older consumers and those with chronic illness. For Allied Heath clinicians, demonstrated extensive specialist knowledge of frail older consumers and those with chronic illness.
- Demonstrated knowledge and understanding in applying research and evidence-based practice to the workplace with the ability to think critically, problem solve and link outcomes to the principles of continuous quality improvement in the clinical speciality field.
- Demonstrated high level of interpersonal and communication skills, including written, verbal and computer skills and an ability to work collaboratively in a multidisciplinary team.
- Proven clinical leadership capabilities with the ability to provide consultancy advice, education and support to individuals, their families, allied health and nursing staff, and other health professionals.
- Demonstrated ability to lead family/ case conferences, discharge planning and coordinate transfer of care for complex presentations across the continuum of care. This includes a demonstrated knowledge of the eligibility criteria and referral process for post discharge options of care.
- Current unrestricted drivers' licence and the ability to maintain. Ability and willingness to drive/travel as the role requires. This may involve driving long distances.
