The Role
We’re currently looking for a Team Leader, Connections Program to:
- Provide specialist Drug and Alcohol (D&A) advice and support to the Connections Program, while maintaining a small clinical case load of people with D&A use disorder who are leaving custody and returning to the community.
- Provide clinical leadership that facilitates the ongoing professional development of the Connections Program clinicians in D&A practices. The role will be responsible for leading ongoing development and implementation of evidence-based D&A interventions by providing expert clinical advice.
- Coordinate and allocate caseloads, and support the clinicians, to ensure care plans are developed with necessary supports tailored for individual patient needs, and that patients linked to appropriate health and welfare service providers post release.
- Partner across Justice Health NSW clinical streams, other NSW government and non-government organisations (NGOs) to enable the transfer and continuity of patient care. Leading practice change and participating in workforce and service development initiatives and participate in service planning and policy development for D&A.
