Where you'll be working:
This position is based at Newcastle Community Health Centre with the opportunity to work at other locations within the service. The social work position is based with the larger Transitional Aged Care Program multi-disciplinary team, and you will play a vital role in supporting clients in this program. You will engage with a broad range of clients and their family and carers which will enrich your practice. We pride ourselves on our collaborative and inclusive culture. About the role:
The successful applicant will provide high level social work services to clients of the Transitional Aged Care Program but will also work collaboratively with the social worker to support clients across other programs within the service.
The social worker will report to the Allied Health Service Manager for HITH RITH and TACP. Clinical supervision will be provided by the network of social workers across the District.

The applicant will have a commitment to supporting and empowering clients and demonstrate empathy and advocacy skills. The applicant will demonstrate resilience and an ability to be flexible and adaptable.
Requirements:
- Bachelor in Social Work or equivalent with eligibility for membership with Australian Association of Social Work (AASW).
