About the role
We are seeking a dynamic, energetic and experienced Social Worker with a passion for social work and commitment to work within the Mission in Action of HammondCare. You will have strong experience within a hospital or health care environment, and be ready to step up into a leadership role within a small team. You will be motivated to support our busy and dedicated Social Work team based at Braeside Hospital in Prairiewood, in collaboration with the Manager of Social Work Services, to ensure a high standard of skilled, effective and timely Social Work interventions for our clients and their carers. You will bring a solid understanding of social work practice, quality improvement and professional supervision. You are ready for a new challenge in a Level 3 role and consideration will be given to either a full-time or part-time job share position for the right candidate/s.
Our health services at Braeside Hospital work within multi-disciplinary teams and focus on inpatient and ambulatory rehabilitation, inpatient older persons mental health, and inpatient and community palliative care. This role will carry a clinical caseload and may rotate depending on service needs. The Braeside Hospital catchment supports clients from a highly diverse cultural and linguistic environment, with SWSLHD having more than 55% of people speaking languages other than English at home. An aptitude for working with diverse complex client populations is essential. HammondCare’s focus is on enhancing the quality of life of our clients, with a holistic approach that respects diversity, and embraces physical, psychological, emotional, spiritual and practical issues.
We are seeking someone with the focus and energy to assist the Manager of Social Work Services to provide support, development, supervision and leadership to the social work team, ensuring compliance with Policies, Guidelines and relevant legislation, and the continuous quality improvement of our services. This includes a high quality, evidence based social work service and skilled practitioners, who convey the organisation’s Mission and Values, and operate within the ethics and values of the AASW. You will also work with the Social Work and Allied Health leadership from HammondCare’s other hospitals and health services towards service and staff development, and ongoing quality improvement.

Essential Criteria
- Recognised tertiary qualification in Social Work with eligibility for AASW membership. Overseas qualifications must provide AASW Overseas Assessment number. Current working rights for Australia i.e. citizenship, permanent residency or working visa.
- Commitment to engage and align with HammondCare’s Mission in Action.
- Experience in a hospital or health care environment (minimum 5 years), with demonstrated high level clinical skills providing a wide range of complex Social Work interventions - preferably including rehabilitation, palliative care / life limiting illness or mental health.
- Demonstrated use of advanced level professional judgement and ability to lead and independently problem-solve set priorities in complex situations.
- Superior interpersonal, communication and reasoning skills necessary to ensure high clinical practice standards and build strong team practice and consistency, using a variety of media – oral, written and digital.
- Self-motivated and effective skills in time management, organisation, and managing competing demands, including balancing a caseload, staff support and service leadership.
- Knowledge, understanding and skills in provision of professional and clinical Social Work supervision and clinical leadership - experience preferred.
- Experience developing and leading successful quality improvement projects.
- Willingness to travel and work between HammondCare’s sites and services as required.
Desirable Criteria
- Experience in a Level 3 Social Work position
- Experience in the use of NSW Health Electronic Medical Record (EMR) or similar
- Current unencumbered Drivers Licence and safe driving record.
- Bilingual and / or experience working with culturally and linguistically diverse communities
