Are you a motivated Registered Nurse with a passion for community-based care? Taree Community Health Service is seeking an enthusiastic RN to join the Transitional Aged Care Program (TACP) – a vital service helping older patients regain their independence following a hospital stay.

About the Role
As a TACP Registered Nurse, you will:

Deliver efficient and responsive care in the community to patients recently discharged from hospital
-
Undertake intake assessments, case management, and coordination of ongoing care arrangements

Assist patients to regain functionality and mobility

Collaborate closely with a multidisciplinary team including RNs, ENs, Physiotherapists, Occupational Therapists, and Domestic Support staff

Provide leadership and guidance to team members across both Taree and Forster locations
You will be part of a wider Aged Care Services team which includes ACAT, Psychogeriatric/Dementia Services, Podiatry, Speech Pathology, Occupational Therapy, and Physiotherapy.

You are a proactive and compassionate Registered Nurse who can work independently in a community setting and thrives in a team-based environment. You have:

Strong communication and interpersonal skills

Confidence in liaising with both patients and multidisciplinary colleagues

The ability to lead and support fellow nurses and allied health staff

A passion for person-centred care and aged health services
