About the role:

TACP Moree provides support to clients with activities of daily living, wound care management, assistance with exercise programs, and education to help them remain in their homes. Additional services may include helping clients with shopping or providing transport to and from medical or health-related appointments. The successful applicant will manage care coordination with support from an Assistant in Nursing (AIN) and Enrolled Nurse (EN) as part of the care team. Clients also have access to input and assistance from Physiotherapy and Occupational Therapy services.

he service is community-based, offering personalised care to elderly clients to help them maintain their independence and avoid unnecessary hospitalisation. By working directly with individuals in their own homes, the service provides tailored support that addresses each client's unique needs, promoting both physical and emotional well-being. This proactive approach reduces the need for emergency care, improves quality of life, and supports seniors in living with dignity and self-sufficiency.
