The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
As a Physiotherapist, you will work across a diverse caseload providing person-centred, evidence-based care to clients in their homes and local settings. You'll collaborate with a multidisciplinary team to support clients with acute, chronic, or post-operative needs—helping them to regain function, improve mobility and enhance their quality of life. You will bring to the role excellent communication, documentation and time management skills and the ability to work independently and flexibly across various settings

What you will bring to the role
- Degree in Physiotherapy and Current National Physiotherapy Registration with the Australian Health Practitioner Regulation Authority (AHPRA)
- Level 1: newly qualified Physiotherapist with less than three years clinical experience; Level 2: a minimum of three years clinical experience as a Physiotherapist and the ability to work with minimal supervision and experience providing community health and/or aged care physiotherapy services.
- Demonstrated effective written & verbal communication skills.
- Demonstrated effective time management skills.
- Demonstrated ability to work effectively in a multidisciplinary team.
- Demonstrated commitment to professional development and demonstrated knowledge of Quality Improvement & Evidence Based Practice.
- Current Driver's Licence and willingness to use for work purpose travel. This may include driving long distances across the LHD.
