About the role:
This role involves providing holistic care to palliative care clients, including end-of-life management, primarily through home visits. You will offer supportive care to clients, their carers, and families, encompassing physical care, medication management, emotional support, assistive equipment, wound care, and coordination with the client’s General Practitioner to ensure optimal care.
We are looking for a candidate with strong time management and excellent communication skills, both verbal and written, and the ability to manage their own workload. The ideal applicant should be adaptable to complex situations, possess a flexible approach, and demonstrate empathy when working with clients facing challenging circumstances. In this role, you will be supported by the Nurse Unit Manager, the local multidisciplinary team, and the Tablelands Sector Palliative Care Team.

Requirements:
Our successful candidate will possess the following:
- Current Authority to Practice with AHPRA and qualifications and / or demonstrated significant experience in working with Palliative Care clients.
- Current unrestricted driver’s license and the ability and willingness to travel for work purposes which may involve driving long distances.
