As an integral part of the hospital’s multidisciplinary team, you will contribute to the assessment, intervention, and discharge planning processes to promote safe and timely patient transitions from hospital to home or community-based care.

To be successful in this role, you will have:

  • Demonstrated experience in acute care or hospital-based settings, with knowledge across various clinical areas.
  • Strong understanding of community supports and services that facilitate safe and effective hospital discharges.
  • Excellent communication, collaboration, and problem-solving skills, especially when working within a fast-paced, multidisciplinary environment.
  • A commitment to delivering patient-centred care and contributing to positive health outcomes.
