In this pivotal position, you'll join an innovative model of care established in 2024, delivering tiered virtual wound consultations alongside scheduled in-person clinics. While based at Manning Hospital, you'll extend your reach to rural communities via outreach and telehealth, helping reduce preventable hospital admissions and emergency presentations.

What You’ll Do
- Lead wound care clinics, triage referrals, and deliver early interventions
- Collaborate with medical, nursing, and allied health teams to ensure integrated care
- Mentor and build capacity among regional clinicians
- Use virtual platforms to consult with patients and document clinical outcomes
- Drive service improvement aligned with strategic targets, including a 10% reduction in chronic wound admissions by 2027
What You’ll Bring
- Current registration as a Nurse Practitioner (or working towards this status via a Transitional NP pathway)
- Advanced clinical decision-making skills and experience in wound care (or strong willingness to develop in this specialty)
- Passion for delivering equitable care to rural and underserved populations

- Comfort with or willingness to learn digital health systems and virtual care
