Step into a leadership role within a passionate and skilled team delivering care to older people transitioning from hospital to home. The Transitional Aged Care Program (TACP) offers short-term care, low-intensity therapy, and holistic support. You’ll work alongside a diverse multidisciplinary team of administration, allied health, and nursing staff based across our Muswellbrook, Kurri Kurri, and Singleton Community Health Centres in the scenic Hunter Valley region.

About the role:
As the TACP Nurse Manager, you will lead a multidisciplinary team with a strong clinical and operational focus. This is a rewarding leadership role where you will:

Provide strategic and clinical oversight of service delivery

Lead policy and operational directions for the TACP

A typical week in this role involves coordination across multiple sites, clinical supervision, operational planning, and stakeholder engagement. You’ll play a pivotal role in ensuring continuity of care and successful transitions for older patients from hospital to home.
Position details:
- 5-day roster with on-call responsibilities
- Travel between sites is required
- Work base is negotiable within the service footprint

Essential:

- Registered Nurse with current registration (or eligible for same) with AHPRA.
