About the Role
We are seeking a dynamic leader for our Social Work team at Braeside Hospital and South-Western Sydney community palliative care services. This leader will manage and deliver Social Work Services, ensuring skilled and timely interventions for clients and their carers. The role includes developing a high-quality, evidence-based social work service and supports practitioners who embody the organisation’s Mission and Values. The Manager will report to the Director of Allied Health and will work within a multi-disciplinary context, supporting staff in diverse cultural and linguistic environments, and will also maintain a client caseload.
Responsibilities
- Ensure high standards of care and practice, maintaining the Mission and standards of HammondCare and complying with relevant policies and guidelines.
- Provide support, guidance, management, and leadership to the Social Work team, fostering continuous quality improvement.
- Maintain a part-time caseload, providing effective social work services and collaborating with the multi-disciplinary team.
- Develop and maintain strong links with internal and external stakeholders to support seamless client transfers and effective healthcare services.
- Formulate, implement, monitor, and evaluate new procedures, programs, or policies based on best practices.
- Promote understanding of the Social Work role and ensure participation in education programs.
- Facilitate regular team meetings and opportunities for staff feedback and service development.
Operational Management
- Provide leadership and direction to Social Work staff, addressing service delivery trends and issues.
- Develop and implement Social Work Service Development Plans.
- Working with the Director of Allied Health and Allied Health Leaders, contribute to Allied Health development plans and operational policies.
- Participate in service planning and monitoring within clinical units.
- Ensure compliance with internal and external reporting requirements.
- Monitor and approve income and expenditure within delegated authorities.
- Contribute to and initiate research activities to advance Social Work practice.
About you
- Recognised tertiary qualification in Social Work and eligibility for AASW membership. Overseas qualifications must include AASW Overseas Assessment number.
- Minimum 5 years post-graduate social work clinical experience, preferably in palliative care, rehabilitation, or mental health, across inpatient, outpatient, and community settings.
- Proven ability to lead and supervise a Social Work team. Experience as Team Leader/Senior Social Worker is desirable.
- Demonstrated management skills in service planning, decision making, quality improvement, resource management, and staff leadership.
- High-level communication and interpersonal skills across diverse stakeholders.
- Effective time management and organizational skills.
- Knowledge of Health, Aged Care, and NDIS sectors, including policies, guidelines, and legislation. Familiarity with NSW Health Policies/Guidelines is desirable.
- Strong administrative and computer skills. Experience with NSW Health Electronic Medical Record or similar is desirable.
- Experience working in culturally and linguistically diverse environments.
- Current working rights in Australia (citizenship, permanent residency, or appropriate working visa).
- Current unencumbered Driver's Licence and safe driving record. Willingness to travel across the region and work between HammondCare’s sites as required.
