Integrated Care Coordinator. South Eastern Sydney Local Health District

Selection Criteria

1. Current Authority to Practice as a Registered Nurse, Nursing and Midwifery Board of Australia
2. Minimum five years full time equivalent post graduate qualification experience and a post graduate qualification in a relevant field of practice
3. Demonstrated experience working with and extensive knowledge of the broader health and social issues for people with mental health and/or drug and alcohol concerns
4. Demonstrated ability in communicating effectively with multiple care providers including general practitioners, other primary care providers, and members of multidisciplinary teams as required
5. Demonstrated effective computer skills in using electronic medical records and the Microsoft suite of products(Outlook, Word, Excel, PowerPoint)
6. Demonstrated clinical expertise and the ability to apply clinical expertise in person-centred care planning
7. Demonstrated ability to work independently and within a multidisciplinary team
8. Current unrestricted Class C NSW Driver's Licence
