  1. Recognised Degree or other qualification deemed appropriate by the employer or relevant experience in providing case management or support planning in community-based health and social care settings.
  2. Demonstrated ability to develop strong client and stakeholder relationships, including the ability to actively seek feedback from clients to shape individual care plans.
  3. Demonstrated excellent communication and interpersonal skills including the ability to actively listen, clear verbal communication and effective written communication.
  4. Effective organisational skills and the ability to work independently and as part of a broader multi-disciplinary team.
  5. Demonstrated experience and skills in relation to community engagement, and in understanding people and their stories.
  6. Demonstrated knowledge of (or willingness to learn) local Fairfield LGA organisations and the services and programs they offer to the local community.
  7. Demonstrated ability to be resourceful and proactively seek out and access a wide range of community-based services, resources, and support options to meet individual’s needs.
  8. Demonstrated commitment to improving the health of vulnerable and marginalized communities including a sound understanding of diversity, culturally safe practice and impacts of stigma.
  9. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
