What You Will Be Doing
We are seeking a passionate and experienced Clinical Care Coordinator to join our dynamic outpatient services team, supporting the ENT (Ears, Nose & Throat) and Urology specialties in a job share role working Thursday and Friday.

Working closely with your co-coordinator and a wider multidisciplinary team, you’ll play a vital role in improving patient access and continuity of care.

What your day-to-day will look like:

  • Coordinating seamless transitions from General Practice to specialist care
  • Managing referrals, waitlists, and appointments using triage guidelines
  • Supporting the integration of Telehealth and alternative service delivery models
  • Strengthening connections between primary care, acute services, and community networks
  • Acting as a central liaison across departments to ensure continuity of care post-discharge
  • Analysing data and reporting on patient metrics to inform quality improvements
  • Contributing to education programs for primary healthcare providers

 

This role requires strong communication, stakeholder engagement, project management, and data analysis skills, with a genuine commitment to improving patient outcomes.
