What you'll be doing
  • Ensure optimal service provision for patients/clients and carers through the promotion of best practice including the appropriate escalation of care, for the facilitation of patient discharges via the discharge/transit lounge.
  • Under the guidance of a Registered Nurse, provide safe, compassionate, person-centred care in collaboration with the multidisciplinary care team to achieve quality care outcomes for patients and clients on discharge from the facility.
  • Utilising effective communication work collaboratively with members of the multidisciplinary care team to provide patients/clients and carers discharge instructions, medications, and paperwork.
  • Act as an advocate for the specific needs of patients/clients and carers including those from Aboriginal & Torres Strait Islander and people from multicultural communities.
  • Maintain and promote a professional standard evidenced by ethical nursing practice, adherence to the policies and procedures of the FWLHD Nursing and Midwifery Service, and the appropriate legislation and statutory regulations.
  • Effectively manage resources relevant to the service.
  • Work with a high level of autonomy regarding the decision-making process in relation to the patients care and facilitating their discharge from the hospital.

  • Current registration as an Enrolled Nurse (without notation) (Division 2) with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Demonstrated application of professional and ethical boundaries to the delivery of patient care in the context of a multi-disciplinary team.
  • Demonstrated application of evidence-based practice and critical thinking to advocate for and implement individualised person-centred care.
  • Demonstrated capacity to identify and resolve conflict situations by managing own emotional responses and interacting with others in a personable and professional manner.
  • Demonstrated commitment to work and partner with Aboriginal and Torres Strait Islander patients and families and employees to enhance health outcomes for Aboriginal and Torres Strait Islander clients.
  • Evidence of participation in own professional development and commitment to maintaining competence and enhancement of skills, and willingness to apply and share professional expertise with colleagues.
  • Demonstrated understanding of evidence-based best practice and its contribution to changing practice and policies within the organisation.
  • Demonstrated effective interpersonal skills, excellent written and verbal communication skills within a multidisciplinary team.
