What you'll be doing
The Community Aged and Extended Care Coordinator position is responsible for providing leadership, support and coordination for the programs within the Aged Care and Chronic Disease Management  portfolio.  The position has key leadership and change management responsibilities to develop and integrate SNSW LHD Community Aged and Extended Care service models of care.



Selection Criteria

  • Relevant tertiary qualifications in a health related discipline or equivalent experience.
  • Demonstrated experience with the co-ordination, driving targets and performance within defined resources across a complex and diverse organisation.
  • Demonstrated experience in leading groups and facilitating educational forums.
  • Demonstrated competency in data entry, high level analysis of data reports, education re data systems and ensuring clinician compliance including planning and executing remediation activity when necessary.
  • Demonstrated experience in budget management.
  • Demonstrated understanding of the principles of change management and experience in their application to influence practice change, implement models of care and service delivery.
  • Demonstrated expert knowledge, skills and competence in providing Aged Care & Chronic Disease Management programs and related guidelines and/ or legislation.
  • Current unrestricted drivers' licence and the ability to maintain. Ability and willingness to drive/travel as the role requires. This may involve driving long distances.
