Community Supportive Care
To be considered for this position you must be an employee of South Eastern Sydney Local Health District and its Affiliated Health Organisations.
The Role
Plan, coordinate, and implement in-depth levels of clinical expertise and extensive specialised knowledge within Nutrition and Dietetics in order to provide high quality Dietetic care to patients consistent with NSW Health, South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.
This is a rotating position based in the St George Hospital (SGH) Department of Nutrition & Dietetics. The initial allocation will be working with the SGH Community Supportive Care Service. The Dietitian will have responsibility for providing clinical care and care coordination to people living within SESLHD who have end of life and palliative care needs and who are in the last 12-24 months of life. The Dietitian will support these patients through the Community Supportive Care Services (CSCS) model at SGH.
The position will assist with triage of patients to the appropriate community or outpatient services. Once accepted to the CSCS, the Dietitian will provide comprehensive health assessments, therapy, education and support to patients and their families. Within this role, there will be a requirement to undertake clinical reviews across clinic, community and virtual care settings, either alone or in conjunction with a multidisciplinary team. This role will also focus on building the capability of inpatient clinicians by supporting them to identify unmet palliative care and end-of-life needs, promoting early referrals to specialist palliative care services, and acting as a conduit for communication between the ward-based clinician and the palliative care multidisciplinary team (MDT).
The hospital is currently undergoing redevelopment and in 2026 will open a brand new Kensington Street Building (KSB), bringing together a range of day only, ambulatory and outpatient services that are currently being delivered from a number of locations around the hospital campus. This will improve accessibility for patients and their carers; particularly for those less mobile or who have chronic health conditions. The KSB will also include three levels of inpatient beds. The St George Hospital Redevelopment will provide staff, patients and carers with a first-class facility, designed to meet the health needs of our community, now and into the future.
The vision for South Eastern Sydney Local Health District (SESLHD) is ‘exceptional care, healthier lives’. SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it.
