Where you'll be working
Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Cumberland and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. What you'll be doing
Provide safe compassionate person-centred care through assessment, planning, implementation and evaluation of nursing care in collaboration with individuals and the multidisciplinary health care team to achieve quality care outcomes for clients.

Essential Criteria:

  • Registration:  Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA)  

Selection Criteria:

  1. A minimum of three (3) years full-time equivalent experience working in Palliative Care and/or End of Life care Nursing. Relevant post graduate qualifications preferred.
  2. Demonstrated skills and knowledge in case management, including care co-ordination for people living with a life limiting illness, using advanced clinical skills in patient/client assessment and care planning.
  3. Demonstrated ability to provide leadership in the provision and development of clinical practice using evidence-based research, demonstrated understanding of current NSW Policies, National and Standards related to Palliative Care.
  4. Demonstrated ability to communicate effectively with clients, carers and a diverse range of health care providers including those in government, non-government and community organisations.
  5. Demonstrated ability to work within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion with an ability to identify and improve opportunities to enhance continuity of care between the acute sector, primary care providers and community-based services.
  6. Demonstrated ability to work with consumers, families and carers in a time of high levels of family stress.
  7. Demonstrate ability to utilise information technology within a variety of applications and willingness to undertake training.
  8. Current Drivers Licence valid within Australia and willingness to use for work purposes.
