The Clinical Nurse Consultant National Comprehensive Dementia Care (NCDC) pilot is a Registered Nurse who has responsibility for triaging new referrals to the NCDC pilot clinic with oversight from consultants; for facilitating bookings, follow-up appointments and referrals, billing codes and clinic function in liaison with the administration staff; to assist with identifying patients eligible for inclusion in the Australian Dementia Network (ADNeT) Registry; to attend multi-disciplinary cognitive disorders case conference to input into consensus diagnosis and formulation of management plans; to maintain communication with relevant community nurses regarding clients of the pilot clinic whom may require additional review; to contribute to ongoing service improvement and quality improvement, including through championing and supporting ADNeT Registry participation; to contribute to promotion of clinic activities through liaising with local General Practitioners and the Primary Health Network as required; and to regularly liaise with Coordinators based at other pilot sites.

Established in 1958, The Sutherland Hospital campus offers a comprehensive range of inpatient and community services to around 226,000 residents of the Sutherland Shire, colloquially known as ‘The Shire’. Part of the South Eastern Sydney Local Health District (SESLHD) and located in Caringbah, the hospital campus is close to some of Sydney’s great beaches, restaurants, bars and cafes at Cronulla and is close to the beautiful Royal National Park. At Miranda, just a short walk away from the hospital, there is a Westfield Shopping Centre and cinema complex.
The ideal candidate will have:
- Current Authority to Practice as a Registered Nurse/Midwife, Nursing and Midwifery Board of Australia and current NSW Drivers License (if required by service).
- Minimum five years full time equivalent post qualification experience, with at least three years full time equivalent experience in the specialty field and post graduate qualifications relevant to the field of practice or such other qualifications or experience deemed appropriate by the Health Service.
- Proven clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing/midwifery practice.
- Demonstrated effective communication and inter-personal skills across a variety of mediums and an understanding of the practice development framework.
- Proven ability to implement changes and develop direction in clinical practice by the utilisation of evidence based research.
- Demonstrated skills in strategic and clinical service planning including an understanding of organisational behaviour, change management and cultural change processes.
- Demonstrated skills in clinical teaching, competency in the use of contemporary information systems and experience in the development of specialty education programs.
- Demonstrated clinical expertise in client-centred consultancy with the ability to work independently
